Job Title:   Life Coach

Description:   The primary function of the Life Coach is to contribute to the development & implementation of Individual Rehabilitation Plan (IRP), to work proactively with the consumer toward achievement of IRP strategies and objectives, and to complete accurate documentation for billing and licensing purposes. Life Coaches are instrumental in the consumer’s orientation and housing search process, and work to ensure the consumer maintains a healthy home environment.

Location:   Monmouth County

Responsibilities:  

  • Engages the consumer and his/her family in a proactive manner to learn how each person wants to live.
  • Works with each consumer to complete the KSR as part of IRP development
  • Provides the required units of service in the areas of therapeutic skill development, services and support, in accordance with each consumer’s IRP
  • Documents observations, information and services provided to the consumer in progress notes
  • Communicates observations and information about the consumer to the Team Leader, Team RN, Clinical Director, and/or Property Manager, as applicable
  • Provides education and support for each consumer surrounding the maintenance of a healthy and sanitary living environment
  • Completes vehicle logs, and provides assistance and feedback to the Property Manager in facility management
  • Provides transportation, counseling and recreational/socialization support
  • Provides crisis intervention services
  • Ensures that all chart documents are filed in a timely and accurate manner
  • All other duties as assigned or as necessary

Qualifications:

  • A Bachelor’s Degree in a helping profession discipline is required
  • Valid NJ Driver’s License with a clean driving record is required
  • Working knowledge of and ability to work on computers is mandatory

Salary: $47,000-$50,000/yr based on qualification

Job Title:   Assistant Director – Clinical Services

Description:     The primary responsibility of the Assistant Director – Clinical Services is to serve as liaison between the CSS direct care staff and clinical/RN staff, ensuring continuity of service provision across the agency. This position acts as the Director or Clinical Administration or “Clinical Director’s” (CD) designee to ensure that all CSS documentation is completed accurately and on time, in accordance with agency, funder, and licensing body requirements.

Location:   Monmouth County

Responsibilities:  

    • Ensures clinical service coordination and assessment of admitted and assigned individuals
    • Ensures timely CSS enrollment and physical move-in of assigned individuals
    • Works with QA to ensure successful achievement of clinical and organizational objectives
    • Completes all assigned CSS and agency documentation in a timely manner
    • Completes clinical and supervisory activities and documentation as needed for non-clinically-licensed staff
    • Provides input to Clinical Director relevant to staffing observations and clinical service provision
    • Partners with CD to ensure timely completion of CSS Documentation across the agency
    • Assists Clinical Director as needed to complete CSS Admission and CSS Termination forms
    • Carries out specific, short-term IRP activities as assigned in UR meetings
    • Serves as a key member of QA committee
    • Rotate on-call responsibilities with Clinical Director
    • All other duties as assigned or necessary

Qualifications:     

  • A Master’s Degree from an accredited college or university in Social Work, Rehabilitation Counseling, Psychology, Counseling, or other related behavioral health/counseling program is required
  • An active clinical license, such as a LCSW or LPC, is required
  • Excellent skills in Microsoft Office, experience working with an EHR, and strong verbal and written communication skills is mandatory
  • A valid NJ Driver’s License with a clean driving record is required

Salary:    $70,000-79,000/yr. based on experience

Job Title:   Director of Clinical Administration

Description:   The Director of Clinical Administration is responsible for oversight of all Community Support Services (CSS) Programs, clinical services, and IRP development. This position is a key member of the QA Committee, chairs the Utilization Review Team meetings, leads the IMR Team, and supervises CSS Team Leaders. 

Location:   Monmouth County

Responsibilities:  

  • Oversees all aspects of CSS Programs
  • Ensures clinical service coordination and assessment of current and referred individuals
  • Works alongside the DON to ensure completion of all clinical documentation
  • Partners with DON and CSS teams to develop IRPs
  • Works with QA Dept. to ensure successful achievement of clinical and organizational objectives
  • Conducts and documents supervision meetings in accordance with agency, funding, and licensing requirements.
  • Participates in the training, orientation, and professional development of CSS Program staff
  • Serves as the agency’s Illness Management and Recovery specialist
  • Completes all required CSS and agency documentation in a timely manner
  • Serves as a key member of the QA Committee and chairs the UR Team meetings
  • Provides on-call support for staff 24 hours a day

Qualifications:

  • A Master’s Degree from an accredited college or university in Social Work, Rehabilitation Counseling, Psychology, Counseling, or other related behavioral health/counseling program is required.
  • An active clinical license, such as a LCSW or LPC, is required.
  • This individual is required to have a working knowledge of computers as well as excellent verbal and written communication skills.
  • A valid NJ Driver’s License is required.

Salary: $90,000/yr
