Support Services

While Declarations provides housing solutions, we also provide support services independent of residential programs. These services may be provided to individuals living in apartments, town homes and other residences leased in the individual’s name or to Declarations. Assistance is provided at various times throughout the week on an individualized basis to help individuals live as independently as possible. This includes meal planning, general health care management, grocery shopping, scheduling medical appointments, arranging transportation and paying bills. Social services such as job training, alcohol and drug abuse programs and case management are provided as needed.

HOUSING
Declarations assists the individual with locating housing in the environment of their choice and arranges for any necessary financing.

GAINFUL EMPLOYMENT
Available employment services include job searching, assistance with training and adjustment, and consistent follow-ups to ensure success. Once a person is successfully placed in a job, Declarations provides support with ongoing training, re-training for potential advancements, assistance in seeking a new job and placement services.

DAILY LIVING SKILLS
Support provided include meal planning, general health care management, grocery shopping, scheduling medical appointments, arranging transportation and paying bills.

MEDICAL CARE
Although Declarations has no medical staff, we assist individuals in finding the appropriate medical care through community providers. Declarations staff will ensure that any and all required medical attention is sought and conduct follow-ups.

BEHAVIORAL HEALTH CARE
Psychiatric follow-ups and counseling/therapy are arranged by Declarations as needed.

TRANSPORTATION SERVICES
Transportation support services are offered to enable individuals to gain access to their community. We empower these individuals who have been relying on staff for their travel needs by teaching them how to access public transportation. Our staff will accompany the individual until they have become acquainted with the process and can travel independently. More informal support, such as help with scheduling, is available after that point.

OBTAINING FINANCIAL BENEFITS
Declarations provides assistance with applications for rental/housing assistance, disability payments or government medical insurance.
