Individual Services for People with Disabilities
Declarations’ residential programs are dedicated to the well-being, health and safety of the people we serve. At Declarations, we do not expect our consumers to spend years waiting for an opening in one of our residential programs. Declarations designs and develops new programs when needed. Each program is designed with the input of the consumer and his/her family. Through rehabilitation planning, life skills training and community integration, we are driven to encourage each individual to reach his or her highest level of independence and quality of life. Those who we serve receive varying degrees of supervision and assistance based on their specific needs. Goals are developed annually with input from the individual and significant others (family and friends), care workers and staff. They may be related to any category of an individual’s life and are designed to meet that person’s needs, hopes and dreams.
GROUP HOMES
Small group-living arrangements in single family homes for four to five individuals with staff on-site 24 hours a day. Support and training, including assistance with daily living, skills training and community integration activities, are provided daily. |

SUPERVISED APARTMENTS
Individuals live by themselves or with other individuals in an apartment owned/leased by Declarations. Staff is on-site 24 hours a day.
SUPPORTIVE HOUSING
Housing is provided with 25 hours of support services per week based on individual strengths, preferences and needs. Staff on-call 24 hours a day.
INDEPENDENT LIVING
Individuals live in their own apartments with no on-site supervision and receive up to 10 hours of support service per week. Staff is on-call 24 hours a day. Assigned counselors meet with the individuals each week to provide assistance and to work on goals.
IN-HOME SUPPORT
An alternative to out-of-home placement, this program allows individuals to live at home with family. The types of supports provided can be tailored to meet the needs of the individual receiving services, as well as their family and friends. Services are provided based on the number of hours needed at times when family and friends are not available or when more specialized training is required. |
